The SIJ is an incredibly strong joint that is perfectly designed to lock into place, providing a stable base for all movement. However, too much stability reduces the efficiency of movement and therefore we need to understand when the SIJ is optimised for stability and when it is optimised for movement. This is why we see the SIJ dividing clinicians on the contentious issue of whether it moves or not, varying greatly between different schools of thought.

 

Finding the balance between the dynamic nature of the SIJ and the way in which it transfers load is about understanding the intricate relationships of articulation, active to passive energy transfer and symptom provocation. The key to this may well lie in a detailed understanding of the relationship of the non-contractile tissue (ligaments etc) to both form and force closure.

 

Once we understand this we can then apply the principles of loading in a number of different ways through postural sets and positioning to enable form closure; to creating appropriate tension in the myofascial system for force closure; to optimising prescription to get appropriate adaption within the non-contractile tissue. This then enables bridging of the above two concepts

  • How to target adaptations for each subsystem of SIJ function
